Mold Town Council, Sid Matthews Community Awards – now open for nominations

Mold Town Council is asking for nominations to be put forward for the Sid Matthew Awards, which takes place at the Annual General Meeting of the Town Council on Wednesday 11^th May 2016

There are four categories covering outstanding voluntary service and / or outstanding achievement within the community of Mold

1) Outstanding Voluntary Service or Achievements by an Individual

2) Outstanding Voluntary Service or Achievements by a Local Organisation

3) Outstanding Voluntary Service or Achievements by a Young Person

4) Outstanding Voluntary Service or Achievement by a Local Youth Organisation

Anyone who knows of a group or person deserving of the award, nomination forms are attached, or they can be collected from Mold Town Council or downloaded from our website http://www.moldtowncouncil.org.uk/Mold-TC/sid_matthews_awards-11781.aspx

There is no limit on number of nominations you can make, but please ensure you complete a form for each nominee

The awards will be presented by the Town Mayor on behalf of the Town Council in memory of the town's former clerk Sid Matthews who held the post from 1974 - 1988.

All nomination forms need to be returned to Mold Town Council by 1st April 2016.
